  • Develop and implement a home based behavioral and/or academic support plan

  • Provide psycho-education -information to family, care givers, school providers and individuals involved with the child’s care about child’s diagnosis, needs and treatment

  • Work with school system and family to develop school supports and accommodations aimed at increasing academic and social success of the child

  • Participate in Individual Education Plan meeting to support needs of child and provide education to family regarding IEP process and expectations

  • Provide school training and supports to give teachers necessary tools and strategies to increase child’s academic and social success

  • Intensive Case Management

    • Linkage and referral to paid and natural supports

  • Arrange for psychological and psychiatric evaluations

  • Assist in developing crisis plan
