• Continue to assist in implementing behavioral/academic support plans

• Continue to work with school system to implement accommodations and supports to increase social and academic success of child

• Monitor implementation of Individual Education Plan arrange further meetings to make adjustment as necessary

• Monitor student progress in school setting on a weekly basis

• Continued case management services

• Follow up on testing assessments and make recommendations as needed

• Monitor crisis plan
